In order to receive an Ukiah library card, you must...
Children too young to have a library card may check out materials only when accompanied by an approved parent/ guardian who has a valid library card from Ukiah or another Sage system library. A Ukiah library card is accepted at all other Sage system libraries. If you live in the 97880 Zip Code area and you have a card issued by another Sage system library, please give it to us when you sign up for your Ukiah account. We will notify your former library of your change of address so your previous account can be closed. LOST CARD: Upon application, each library patron receives a Ukiah Library Card. If the card becomes lost or unreadable, the patron will have to pay a fee of $5.00 to receive a replacement card. |
The application form is short and easy to fill out. If all the needed information is provided and the automated system finds your name clear for approval, your new library account & card will be ready for use a few minutes after you complete your application.
ADULTS: Bring your current Oregon driver's license or photo ID. If your license/ID card is out of date, you will need to bring two documents: one showing your current address (a current utility bill, rental agreement, bank statement, etc.) and a photo ID (driver’s license, student ID, state ID, passport, etc). You may also be asked to provide other information that will help the system create an account for you. MINORS: Minor children are not required to have photo ID to apply for a library card, but they must have a legal guardian present in order to sign and complete the card application. The legal guardian must sign the application, indicating his/her willingness to assume responsibility for all fees and fines that may be charged to the minor’s account. Unless otherwise requested, the library will keep the minor's library card on file until the child enters 7th Grade. APPLICATION “ON HOLD” If at the time of registration the applicant has outstanding fees and/or fines remaining on an account at another library in the Sage system, the new application will be considered “on hold” until the previous account has been cleared and closed. You may pay outstanding fees and/or fines for sage libraries at our library. |
To open your online library account, follow these steps in order:
1. Have your library card handy. 2. Go to catalog.sage.eou.edu 3. Click on "My Account". 4. Put in all 14 digits of your library card number. 5. Click on "Forgot Password". 6. A note will appear that says you will receive an email directing you to change your password. If you do not get an email shortly, call the library to check your email address on record. Items borrowed using your Sage System library card will be recorded on your online account, and when you log in, you’ll be able to see what items you’ve borrowed and when they are due, in addition to the overall status of your account. From within your online account you will also be able to renew items (extend the time you can keep them) and place reservations (“holds”) on items from home or from any computer with Internet capabilities. You may also activate "history", which will keep a record of all materials checked out on your card. Have you already read this book? Check your history record. NOTE: Keep good track of the user name/library card number and password to your online library account. The Sage Library System will not allow library staff to access your online account or retrieve your user name or password for you.
